The Florida Military Family License Reciprocity Program Application requires the following information:
- First name
- Last name
- Phone number
- Scan of active Photo ID (Passport, Driver’s License, or Government ID)
- Attestation statement: “I hereby attest that the information provided in this form is true, accurate, and complete to the best of my knowledge. I further attest that I am a veteran, active service member, or military spouse and in good standing with the military. I understand that any falsification may result in licensure denial or other consequences.”
